This form is for candidates only.

If you are a Centre, applying on behalf of the candidate, please use the Centre Application Form.

Note: your photo must meet our Photo Requirements or your application will be returned.  The delay may affect your card validity.

Application Details

If you wish to register an Initial Application, Additional Qualification, or Re-assessment, this is normally done by your centre and is included in the price of your course. If however, you wish to apply directly please contact the SWQR team, to discuss the application process and the relevant fees.

Replacement card applications

If your Street Works card has been lost/stolen or damaged, please contact the SWQR team, to make a payment.

Please note, we use an automated payment system, and you will be given a unique reference number when you call us, so please have a means of noting this to allow you to make the payment.

We can only provide replacement Street Works cards that are valid, i.e. the card has not passed its expiry date. (We cannot accept American Express Cards for payment)

Registration Details

SWQR collects information about candidates on behalf of the Department for Transport. This information is used to administer the Register, and to produce SWQR ID cards. Further details can be found on this website.

Street Works cards can be re-registered up to a maximum of 3 months after the card expiry date provided the card fee has been paid. If the card has expired more than 3 months after you apply, or you have not paid the card fee in time, you will need to undertake a new assessment.
