Scotland, Wales and Northern Ireland

Supervisors/Operatives in Scotland, Wales and Northern Ireland are required to re-register with SWQR within 3 months of the expiry date of the existing registration. Each supervisor /operative should complete a ‘Candidate Re-registration form SWQR2’ and return it to the Street Works Register along with their current ID card, one passport and size photograph and the appropriate fee. Late re-registrations can be received up to 3 months after the candidate expiry using the standard (SWQR2) but the re-registration fee will be charged at a higher rate. The expiry date on the ID card will be five years from the original expiry date.

In the event of an ID card being lost or stolen, the Street Works Qualifications Register should be notified immediately. The candidate will then be issued with a ‘Candidate Replacement Form (SWQR3)’. The candidate will complete this form and return it to the Street Works Qualifications Register along with one passport-sized photograph and the appropriate fee.
