Supervisors/Operatives working Scotland, Wales and Northern Ireland are required to re-register with SWQR. Each supervisor /operative should complete a 'SWQR Application Form' and return it to the Street Works Register along with their current ID card, one passport and size photograph and the appropriate fee. Re-registrations can be received up to 3 months before and up to 3 months after the candidate expiry. Once the expiry date has exceeded 3 months candidates can still re-register however the re-registration fee will be charged at a higher rate. The expiry date on the ID card will be five years from the original expiry date.

In the event of an ID card being lost or stolen, the Street Works Qualifications Register should be notified immediately. The candidate will then be issued with a 'SWQR Application Form'. The candidate will complete this form and return it to the Street Works Qualifications Register along with one passport-sized photograph and the appropriate fee.
